St Lawrence Surgery must ensure they meet the needs of all their patients and/or carers if they have any information or communication needs. Your needs (if any) will then be recorded on your medical records and shared with other NHS and Adult social care providers, when you have given consent or permission to do so.
We need to ensure that our patients get information in an accessible way and provide communication support if you need it. So, please do let us know as soon as possible if you have any requirements by telephoning us on Tel. 01903 222900, calling at reception or emailing firstname.lastname@example.org – Practice Care Co-Ordinator.
The practice recognises that our patients may have diverse communication styles and needs. In line with accessibility standards we have a wide range of facilities to accommodate this diversity, such as a hearing aid T-loop, interpreter service, large print practice booklets, lowered reception desks and support for children with additional needs and their families. Please let us know how we can communicate with you better if you have any specific needs.
